This role requires a dynamic leader with a strong background in healthcare, particularly in the field of diabetes prevention and management. The coordinator will be responsible for overseeing all aspects of the program, including developing educational content, community outreach, project planning, evidence-based practice integration, continuous quality improvement, change management, collaboration with hospital leaders, diabetes navigators, and community resources, as well as data collection for program evaluation.
a. Create comprehensive educational materials and resources tailored to individuals at risk of diabetes or those diagnosed with prediabetes.
b. Design and implement engaging workshops, seminars, and educational sessions focused on lifestyle modification, nutrition, physical activity, and stress management.
a. Conduct and oversee risk assessments and comprehensive evaluations for patients at risk of diabetes.
b. Assess, analyze, plan, implement, and evaluate the nursing care for patients at risk for diabetes or diagnosed with prediabetes.
c. Perform point-of-care laboratory testing, including hemoglobin A1C, to monitor patient health status.
d. Provide individual and group patient education through formal and informal education activities.
e. Document patient encounters and interventions accurately and thoroughly, ensuring compliance with regulatory standards.
f. Provide appropriate follow-up care and referrals as needed to support patients in managing their diabetes risk or condition.
a. Establish and maintain strong relationships with community organizations, healthcare providers, and local stakeholders.
b. Conduct outreach activities to raise awareness about the program and engage individuals who may benefit from diabetes prevention services.
c. Collaborate with community partners to promote health equity and address social determinants of health affecting diabetes prevention efforts.
a. Develop and implement strategic plans and objectives for the program in alignment with organizational goals.
b. Coordinate program activities, including scheduling workshops, recruiting participants, and securing resources necessary for successful implementation.
c. Oversee day-to-day operations, ensuring efficient delivery of services and adherence to program protocols.
a. Stay abreast of the latest research and evidence-based practices in diabetes prevention and wellness.
b. Translate research findings into practical strategies and interventions to enhance program effectiveness.
c. Collaborate with interdisciplinary teams to integrate evidence-based guidelines into program activities and interventions.
a. Develop and implement quality improvement initiatives to enhance program effectiveness and efficiency.
b. Lead change management efforts to adapt program strategies and processes in response to evolving needs and best practices.
c. Evaluate the effectiveness of educational materials and adjust content based on feedback and outcomes.
d. Monitor program outcomes and performance metrics, identify areas for improvement, and implement corrective actions as needed.
a. Work closely with hospital leaders, including diabetes navigators and other interdisciplinary healthcare professionals, to ensure seamless coordination of care for individuals with diabetes or at risk of developing diabetes.
b. Collaborate with community resources, such as local clinics, wellness centers, and support groups, to provide comprehensive services for diabetes prevention and management.
c. Collaborate with stakeholders to identify and address systemic barriers to healthy living and disease prevention within the community.
d. Foster a collaborative and supportive work environment that promotes innovation, creativity, and continuous learning.
e. Identify potential funding opportunities and grants to support program expansion and sustainability.
f. Cultivate strategic partnerships with governmental agencies, foundations, and corporate sponsors to secure funding and resources.
a. Provide supervision, training, and mentorship to clinical and non-clinical volunteers participating in program activities.
b. Coordinate student placements and provide guidance to students engaged in clinical rotations or internships within the program.
c. Ensure adherence to program protocols and standards of practice among volunteers and students.
a. Develop and manage the program budget, including forecasting expenses, allocating resources, and ensuring fiscal responsibility.
b. Monitor expenditures and revenue streams, identifying opportunities for cost-saving measures and revenue generation.
c. Collaborate with finance and administrative staff to ensure compliance with budgetary guidelines and reporting requirements.
a. Develop data collection tools and protocols to assess program outcomes and impact.
b. Collect, analyze, and interpret quantitative and qualitative data on participant demographics, program engagement, and health outcomes.
c. Prepare reports and presentations summarizing program evaluation findings and recommendations for quality improvement.
Education
Licensure/Certification
Experience
Knowledge, Skills and Abilities
Strong knowledge of diabetes prevention, wellness promotion, and chronic disease management principles.
Excellent communication, interpersonal, and leadership skills.
Ability to work collaboratively with diverse stakeholders, including hospital leaders, interprofessional healthcare professionals, community resources, and community partners.
Proficiency in data collection and analysis, with experience using statistical software preferred.
Commitment to continuous learning and professional development in the field of diabetes prevention and health promotion.
Valid NC Driver's License: No
If driving a Wayne UNC Vehicle, must be 21 years old and MVR must be approved by Risk Management.
